Impacted Supernumerary Teeth: Diagnostic Evaluations and Surgical Solutions

Impacted supernumerary teeth are either discovered through radiographic exams requested for other reasons or through the clinical observation of anomalies in the development of permanent dentition, such as missed eruption or rotation/inclination of permanent teeth. The seminar aims to illustrate the difficulties of the diagnostic process and to discuss the therapeutic modalities by means of clinical examples. The diagnosis is essentially radiographic: in the rather crowded panoramic radiographs of the mixed dentition, the supernumerary teeth do not always appear at the first glance; sometimes they are not easily distinguished from the teeth of the normal series, also because these are often rotated and therefore present in anomalous projections, precisely due to the presence of the supernumeraries. The position of the supernumerary teeth relative to the roots of normal teeth dictates the surgical access and, albeit rarely, may require the use of three-dimensional imaging to determine the position and shape of impacted teeth. The therapy obviously consists of the removal of supernumerary teeth, which however is not always sufficient to ensure the eruption of normal teeth.
